Most struggling practices rarely fail from a lack of skill. They wear down. 

I’ve noticed certain qualities about practices that truly work, both for the doctor and the patient: 

1. They’re designed with purpose.

2. The economics work. No heroics are needed.

3. They attract the right people—and quietly repel others.

4. Expectations are clear. Nothing important is assumed.

5. They rely on systems, not memory, mood, or personalities.

6. They protect the doctor’s energy—with space to think, recover, and renew.

7. Positive reviews, thank you cards, atta boys, and expressions of gratitude are visible reminders that the work matters.

When these are missing, the gap is filled with self-effort:

Longer hours.
More effort.
Greater emotional investment.

Which works.

Until it doesn’t.

Like the doctor who asked during a recent consultation, “Just show me how to chew off my leg so I can get out of this trap!”
